Signed consent for these procedures is a hospital requirement. This is best completed in advance of attending for a procedure. Patients are sent a consent form to complete and return to Dr Forbes' rooms at the time of scheduling the procedure(s). If there are questions arising from this process, it is advisable to arrange for formal consultation well in advance of the scheduled  procedure.
