Regrettably Dr Geoff Forbes closed his practice permanently on 15 August 2025 due to illness. All patients who were booked to see Dr Forbes for a consult or procedure in 2025 have been contacted. If you have not been contacted directly, please contact your GP.

PATIENT CONTINUATION OF CARE

For all ongoing care, including repeat upper GI endoscopy, colonoscopies and other endoscopic procedures, patients of Dr Forbes will need to see another gastroenterologist. This can be facilitated via your GP.

If you require urgent medical attention, please contact your GP or attend your nearest emergency department.

MEDICAL RECORDS

GP practices received copies of all Dr Forbes patient’s correspondence and reports.

Patients of Dr Forbes can request a copy of their records, in the first instance, from their treating general practitioner (or the GP who made the initial referral to Dr Forbes). Patients can then provide these to another gastroenterologist or other treating medical practitioner for continued care.

All medical records from Dr Geoff Forbes' practice, Forbes Gastroenterology, will be held securely by Glengarry Gastroenterology by Dr Adam Doyle, Dr Oliver Duncan and gastroenterologist colleagues at SUITE 2/64 Arnisdale Rd, Duncraig WA 6023.

To obtain access to your medical records from Dr Forbes practice, please email Glengarry Gastroenterology at admin@glengarrygastro.com.au or by phone 9246 1413.

You will be required to complete a consent to allow Glengarry Gastroenterology to access your records.

Please note: Glengarry Gastroenterology are not taking over the care of Dr Forbes patients but facilitating access to medical records. A fee may be charged.
